Overview

People with opioid use disorder and other complex health needs have limited access to high quality health care and evidence-based self-management education. The Chronic Disease Self-Management Program in addiction-focused primary care settings can help patients build the skills to better manage their own physical and mental health needs. 

This presentation will explore considerations for implementing a chronic disease self-management program in addiction-focused primary care settings, from the perspectives of people in recovery from opioid use disorder and serious mental illness and their primary care providers.

About Presenter

Elizabeth Siantz is an assistant professor in the College of Social Work at the University of Utah. She is principal investigator of a five-year K01 award from the National Institute on Drug Abuse (NIDA) to study the adaptation and implementation of a peer-led chronic disease self-management program for primary care patients with opioid use disorder and serious mental illness. Dr. Siantz is also a VA-funded Implementation Research Institute (IRI) fellow at Washington University St. Louis and is supported by a NIDA Loan Repayment Program for Health Disparities Research.
